Application for Certified Copies of a Marriage License

Expedited processing service is also available for an additional fee for requests ordered on-line via VitalChek.  VitalChek, a third party vendor approved by the CT DPH, allows customers to order certificates from Farmington through either a toll free telephone number or on-line.

VitalChek can be reached either through its website, www.vitalchek.com, or by phone at 1-866-809-0839.  An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.  Only requests ordered via Vitalchek allow customers to pay for certificates with a credit card and to track the status of ordered certificates that are sent by UPS overnight mail.
